For an online video consultation these items are REQUIRED:

• Access to a working VIDEO camera to be able to have an online appointment, this could be a web cam, tablet or smart phone
• A valid, GOVERNMENT ISSUED ID
• FILL OUT the questionnaire 1 hour before your appointment
