For an online video consultation these items are REQUIRED:
- Access to a working VIDEO camera to be able to have an online appointment, this could be a web cam, tablet or smart phone
- A valid, GOVERNMENT ISSUED ID
- FILL OUT the questionnaire 1 hour before your appointment
